If you answer (NO) to a question, your org has serious risk exposure.

Count the number of NO’s as you go!

Explainability.

  1. Can the AI system’s decisions be explained in terms that are understandable to non-technical users?
    Yes / No

  2. Do you provide users with clear reasons behind the AI's decisions or recommendations?
    Yes / No

  3. Is there a system in place for team members to explain the AI’s decision-making process to stakeholders when needed?
    Yes / No

Legal Compliance.

  1. Does the AI system comply with all relevant data protection and privacy laws (e.g., GDPR, CCPA) in the regions where it operates?
    Yes / No

  2. Are data handling practices aligned with industry regulations, including retention and deletion policies?
    Yes / No

  3. Are you familiar with the AI Act? Have you aligned your product to the legislation’s requirements?
    Yes / No

Robustness.

  1. Has the AI system undergone stress-testing to ensure it operates reliably under different conditions?
    Yes / No

  2. Are there safeguards in place to detect and mitigate errors or unintended outputs from the AI model?
    Yes / No

  3. Does the AI system have protocols for regularly monitoring and updating its algorithms to maintain accuracy and reliability?
    Yes / No

Transparency.

  1. Is there clear documentation available describing the AI’s purpose, data sources, and functioning?
    Yes / No

  2. Do users have access to information on how their data is used within the AI system?
    Yes / No

  3. Are you transparent about any biases, limitations, or uncertainties in the AI’s predictions or outputs?
    Yes / No

Governance.

  1. Is there a dedicated team or individual responsible for overseeing AI ethics and compliance within your organization?
    Yes / No

  2. Are governance policies in place that outline roles, responsibilities, and accountability for AI management?
    Yes / No

  3. Does the organization conduct regular audits or assessments of the AI system’s ethical and operational impacts?
    Yes / No
